Overview

     Children First Medical Group, Inc. (CFMG) is continuing the member outreach text messaging program with a new vendor, Luma Health (Luma), in October 2022.

     The text messaging Healthcare Reminders assist our primary care physicians by contacting members via text to close care gaps for wellness visits, immunizations, or screening tests such as lead levels.

     The text messaging Healthcare Reminders will help CFMG improve the preventative health of our members using standard NCQA HEDIS measures as specified by Alameda Alliance for Health.

      The Luma Healthcare Reminders will revolve around HEDIS measures, specifically wellness visits, immunizations, and lead screening to help CFMG achieve the minimum performance level (MPL) of the 50th percentile for each measure.

Program Details

      In October 2022, Luma began reaching out to some of your patients through SMS text messages and or phone calls. 

      Please note that the SMS messages will be sent under the name of Children First Medial Group and the practice name. Participating members will be encouraged to schedule appointments to remain up to date on specific health measures such as Annual Wellness Visits and Childhood Immunizations; but the physician will be the exact name of the member’s physician/practice name at the time the list is generated.

Frequently Asked Questions about Luma

Text message support is available in Medi-Cal threshold languages and will be sent in the member’s preferred language as reported by health plan. Threshold languages for the two counties are as follows:

  • Alameda County: Spanish, Vietnamese, Cantonese, Mandarin
  • Contra Costa: Spanish
Luma will receive weekly updates to the gaps in care directly from CFMG claims data feeds.
The Healthcare Reminders will be texted monthly, on the second Wednesday and Thursday of each month from 10 am-3 pm.
Specific Healthcare Reminders are:
· Well Child Visit (WCV30A) birth – 15 months
· Well Child Visit (WCV30B) 15 months +1 day through 30 months
· Child and Adolescent Well-Care Visits (WCV) 3-21 years
· Childhood Immunizations (CIS-10) birth to 2 years
· Immunizations for Adolescents (IMA Combo 2) -turned 13 in 2025 and received all immunizations by their 13th birthday
· Lead screening – two blood lead level tests by the age 24 months
Yes, in addition to the PCP name and phone number, the message will have a Click to Call feature, which allows the member to click and directly call the PCP. The message will also have a clickable URL for FAQs about the specific Healthcare Reminders.
Each campaign will have an initial sms messaage; then in 4 weeks, if the provider has not reported the campaign being met, a reminder message will be sent to the member.
Each message will have a clickable link to a URL with frequently asked questions (FAQs) about the specific Healthcare Reminder they received via sms messaging. This information will include what do to if the member wants to update provider or phone information.
